Vermont State Health Improvement Plan

The Vermont State Health Assessment is conducted every five years. It is an overview of the health of people in Vermont at a point in time, and the data is used to understand health outcomes and differences across groups. Based on the results, the Health Improvement Plan is developed for the five years between assessments.


The Vermont Department of Health provided Pluck with the State Health Assessment, fully designed, and the copy for the State Health Improvement Plan. These documents would be companion pieces, despite being designed by separate people. My goal was to convey the information in a digestible way while also creating a layout that felt like an extension of the assessment.


I started by pulling out key details from the State Health Assessment: typographic details, like typefaces, weight, and running information, use of hierarchy, and color. I cataloged everything with the intention of mixing and matching styles to narrow down the list as I iterated on various layouts.


In addition to typographic details, we also pulled out visuals. How was the VT Department of Health already conveying their information, and how could we use that in the improvement plan? If someone was reading the improvement plan, they’d most likely be reading the assessment, and forcing a reader to digest information in a whole new way was an unnecessary roadblock. Thus, we decided to use the existing donut charts for our data and lay them out on a dark callout box just like the assessment.


During the iteration process, I designed for both portrait and landscape layouts. The assessment was already in portrait, so it felt natural to design the improvement plan as such. But with each iteration, it became apparent that, with the given information and data, a landscape orientation would best suit our goals.
